Who Needs It
Women who have had children and want breast implants to restore volume lost after pregnancy and breastfeeding often need to combine breast augmentation with a lift. Patients who have had significant weight loss, or older patients who are now seeing the effects of time and gravity, can often get the results they want by combining a breast lift with implants, especially if they want a fuller look in the upper portion of their breasts. Additionally, there are many women who have significant breast asymmetry or problems with the shape of their breasts who will benefit from combining breast lift with augmentation.
Women who want to significantly increase the size of their breasts through augmentation, but choose not to get a lift, may not gain the fullness where they want it, lessening their satisfaction with an augmentation alone. In most cases, breast implants add volume, but won't lift the breasts or create a perkier shape.

Case 211: Breast Augmentation with Lift
This 33 year old woman suffered with significant breast asymmetry, making it very difficult to buy bras or wear swimsuits. She had 4 children which she breastfed. I performed an augmentation with breast lift to improve her symmetry. In these type of presentations, saline implants are wonderful to get good volume symmetry. Because they are filled once they are inside of the patient, it is much easier to determine the right volume. On the right, she has 500 cc and on the left breast 450 ccs. I removed only 16 grams of breast tissue from the right and 75 grams from the left breast. Now she is a pretty 36 D and looks amazing in a swimsuit! This breast lift was performed at our certified surgery center located in Shreveport, Louisiana.

This 28 year old woman wanted her 34 B breasts to be a full C or small D, and to not sag. She had 3 children and had breast fed them all. To achieve her goal, I placed 380cc moderate profile Sientra cohesive gel implants in addition to a breast lift. Her post op result here is 14 months after surgery.

During Dr. Wall Jr.'s examination of the patient, it was recommended that she would also have a short scar breast lift along with her breast augmentation to achieve the look she wanted. The patient has been diligent in the use of post operative scar regimen to allow the scars to fade. I’m always getting asked about my techniques- here ya go: This patient came to see me with huge, low, wide, loose breasts without any upper breast fullness. She actually had very pretty breasts in the past, but childbearing, weight changes, and time, caused them to drop and sag. Breasts like these are traditionally thought of as difficult to lift and manage, and definitely difficult to achieve legitimate upper breast fullness in. The vast majority of plastic surgeons in the U.S. use a very old (inferior pedicle) technique that results in huge (anchor) scars, a boxy appearance, and worst of all, still keeps the breasts low on the chest. Unfortunately, even the best surgeons are hamstrung by their choice of this technique. That operation never made a lot of sense to me so I abandoned it over 18 years ago, in favor of a far superior (pun intended) short scar technique that allows me to create high, full, rounded, beautiful breasts that have 70% shorter scars, have much better healing, and hold up much better over time. Why don’t all surgeons use this technique? It has a very steep learning curve, is difficult to teach, requires a high degree of precision, and until SAFELipo and Breast Tissue Equalization were developed, it was difficult to manage the bottom skin of the breast perfectly. Nowadays, it’s hard to justify performing a “lift” on a patient that results in breasts that are still low and sagging, yet we see it around here every day... Hopefully not from any of our own patients!

Preparing for a Breast Lift With Implants
An important part of the consultation process is describing your cosmetic goals to your surgeon. Based on those goals and a thorough physical exam, the surgeon recommends an approach that should produce results that look great for years to come. That philosophy limits your chances of having to undergo another surgery in just a few years.
Advanced technology and the superior surgical techniques used by our surgeons at The Wall Center minimize the scarring that causes the most anxiety for patients who are combining a breast lift with implants. Advanced silicone gel formulations can also help make incision lines barely visible.
A breast lift with augmentation is usually performed under general anesthesia. Our surgeons use the short scar breast lift technique in almost all cases (also called the vertical or lollipop lift), where the incision is made around the areola and then straight down to the breast crease. This technique eliminates the long scar that runs along the breast crease and creates a better contoured breast shape compared to the traditional anchor-shaped incision.
The surgeon inserts the breast implants through this incision and places them so they are centered around the nipple. Incisions are then sutured and bandaged. The bandages are replaced by a surgical bra after a few days, providing ample support to protect your breast shape.
Rapid Recovery & Results
The breast lift procedure itself doesn't typically produce more discomfort during your recovery than what is normally expected after getting our rapid recovery breast augmentation by itself. The same rapid recovery protocols are used for our breast lift with implants patients, with most patients being able to return to most typical activities the day after surgery. Strenuous work with the chest muscles is avoided for 4 weeks after surgery, but most patients are able to resume exercising just 2 weeks after their surgery. Due to the meticulous way the surgery is performed, most patients experience little to no bruising. Swelling of the breasts usually resolves within 2 weeks, but the implants typically take weeks or months to fully settle into the perfect position, with the goal of long-lasting results and a very low reoperative rate.
In borderline cases, getting a breast lift along with implants can extend your results for many more years than what could be expected with only breast augmentation. These decisions are important and our surgeons are committed to finding the solution that will work best for each individual patient.
