Drawbacks of the Standard Dual-Plane Technique

While the traditional dual-plane technique (commonly in use today) improves breast shape by placing implants partially under the chest muscle, while allowing the central part of the breast to expand and fill, it creates a new set of problems: implants shifting laterally (toward the armpit) and looking unnatural when the muscles flex. The shoulder’s biomechanics can also be compromised. Dr. Wall Jr. and Dr. Holly Wall recognized these shortcomings and created an approach that fixes them.

The Modified Dual-Plane Technique: Developed at The Wall Center

In 2005, Dr. Simeon Wall Jr. created the modified dual-plane technique that makes key improvements to the standard method. His approach leaves the outer part of the chest muscle intact, ensuring the implant stays ideally positioned without dropping out or shifting to the side by the arms. This next-generation technique enhances your breast augmentation results by providing:

  • Increased upper breast fullness that is stable over time, usually over decades, without dropping out or lateralization of the implants, problems commonly seen in traditional dual-plane and over-the-muscle techniques
  • Natural-looking shape, with and without muscle contraction, with an implant that is matched to the width and height of your breast, unlike “2 baseballs in your breasts” sometimes seen with traditional dual-plane techniques

  • Inframammary or transaxillary incisions: Placed discreetly in the breast fold or the armpit to conceal the tiny scar.
  • Keller Funnel® technique: Allows no-touch, small-incision implant placement to reduce infection risk and minimize visible scarring.
  • SAFELipo® of the axilla and lateral chest: Sculpts the entire frame of the breast by sculpting and removing fat near the armpit and along the sides and below the breasts.

Our surgeons use the innovative Keller Funnel® when placing silicone gel implants. The use of the funnel allows for shorter incisions and offers a “no-touch” technique that enhances the safety of the surgery by reducing the risk of infection and shortening surgical times. After the surgeon creates the implant pocket, implants are placed in the lubricated funnel and squeezed through the smaller incision and placed with precision.

The Beauty of Rapid Recovery

Recovery after modified dual-plane breast augmentation is typically quick, with many patients returning to normal activities the evening of surgery and strenuous activities in 2 weeks. Our Rapid Recovery Protocol helps you get back to your life quickly, with minimal discomfort and no need for narcotics.

General Recovery Timeline

Day of surgery: Return home and start gentle stretching exercises that evening. Patients often go out to dinner!

2 to 3 days: Most patients return to work, school, and nonstrenuous activity.

2 weeks: Resume most workouts, avoiding chest exercises. Swelling decreases significantly.

4 weeks: Cleared for swimming, chest exercises, and sports (with surgeon approval).

6 weeks: Swelling fades. Scars are healing. Results become visible.

2 to 3 months: Implants settle into a natural position and feel.

3 to 9 months: Final shape achieved.

Breast Augmentation FAQ
Can I combine breast augmentation with other procedures?

Yes, we often combine breast augmentation with a breast lift, or with other body contouring procedures in a combination surgery known as a Mommy Makeover. This personalized combination surgery can include a tummy tuck and liposuction along with breast enhancement surgery. Even if you’re not a mom, combining procedures can save recovery time and deliver comprehensive results. 

How do I choose the right size and type of implant?

A board-certified plastic surgeon with experience performing thousands of breast augmentations will guide you in choosing between the different types (saline versus silicone), shapes, profiles, and sizes of implants. Our surgeons use 3D imaging and offer in-office sizers to help you visualize your options and feel confident in your decision.

How long do implants last?

Most breast implants last at least 10 to 20 years, but they aren’t considered lifetime devices and may require replacement at some point in the future. We recommend regular check-ins and imaging to monitor your implants. We offer free lifetime ultrasound examination of all breast implants placed at The Wall Center. If issues arise or your preferences change, implant replacement or revision is always an option.

Will I have visible scars?

Breast augmentation requires incisions, so you will have scars, but they should be difficult to detect. Dr. Wall Jr. and Dr. Holly Wall typically use a small, well-concealed incision in the breast crease (inframammary fold) or armpit (axilla), which tends to heal beautifully and is hidden even in swimwear. Scar care protocols are part of your recovery plan to help ensure minimal visibility over time.

What size (cc) breast implants should I get? 

Breast implants are sized by volume (cc), not bra cup size, so the right size for you will depend on your current breast volume, the breast size you want, your body type, and other factors. Our surgeons will recommend the implants best suited for your cosmetic goals and help you envision your new shape using the VECTRA 3D imaging system.

What are my options to pay for breast implants?

During your consultation with one of our plastic surgeons, we will provide a personalized quote for your breast augmentation procedure. To make your goals as accessible as possible, we accept cash, major credit cards, cashier’s checks, and money orders. We also work with multiple financing options for qualified patients who would like to explore flexible payment plans.

Can I breastfeed after breast augmentation?

The vast majority of breast augmentation patients are able to breastfeed after their surgery. Choosing a qualified plastic surgeon experienced in breast augmentation is one of the best steps a woman can take to achieve her desired results. According to prior studies on the subject, there is a 10 to 15% chance you may not be able to breastfeed after breast surgery, although we have found this to be rare, especially when only a breast augmentation is performed.
