During surgery, the skin is separated from the underlying bone and cartilage, the nose is reshaped, and the skin is then carefully re-draped and closed. Open rhinoplasty involves a small incision across the columella—the narrow strip of tissue between the nostrils—along with internal incisions. The surgeon lifts the skin away from the nasal framework, allowing him to directly view and access the underlying structures. It is the preferred approach for complex reshaping, such as:

Structural rhinoplasty refers to any approach that prioritizes rebuilding or reinforcing the cartilage and bone framework that gives the nose its shape and support, rather than just removing or repositioning tissue. This technique is especially important when the nose:
Our surgeons may use grafts, often harvested from the patient’s own cartilage, to reinforce the tip, bridge, or lateral walls of the nose. A structural approach is designed for results that support breathing, look natural, and are long-lasting.
Functional rhinoplasty addresses physical obstructions that impair nasal breathing. The most common problem is a deviated septum, where the wall of cartilage and bone separating the two nasal passages impairs airflow through one or both sides.
Functional rhinoplasty may also address enlarged turbinates inside the nose that can swell and block airflow, as well as other physical causes of chronic congestion, difficulty breathing through the nose, or snoring.
In many cases, our surgeons address functional and cosmetic goals together in a single procedure, improving both the appearance and function of the nose.
It will take about a week after surgery before you will feel comfortable enough to go out in public. Initial swelling can be substantial, but our surgeons can prescribe medication to help ease any pain you may experience. Patients typically are ready to return to work and school in about 2 weeks. You will need to refrain from strenuous exercise for 4 to 6 weeks. Usually, after 2 to 3 months, 80% to 90% of the swelling will be gone, and the long-term results will be more apparent.

Swelling typically takes about a year to fully resolve, with the tip of the nose often being the last area to refine. Once you achieve your final results, they will last indefinitely.
The patient’s nose should be fully developed before nose surgery, around age 15 for girls and at least 16 for boys.
Sleep with your head elevated for at least the first week to minimize swelling and enhance breathing. Propping yourself up with pillows or sleeping in a recliner can be helpful.
