Who Is a Good Candidate for Gynecomastia Surgery?
Candidates for gynecomastia surgery are males in good overall health and at a healthy, stable weight who want to improve chest contour and correct persistent enlargement of the breast tissue or nipples.
Age Requirements
- Most patients are 18 years or older.
- In severe cases, surgery may be considered for patients as young as 16.
Common Concerns
- One or both breasts and/or nipples remain enlarged despite diet and exercise.
- Discomfort or self-consciousness when wearing fitted shirts or going shirtless.
Weight and Body Composition
- Best results are typically seen in patients with a BMI under 30.
- Select patients may be considered with a BMI up to 35.
- Body fat percentage and overall physique are often more important than BMI alone in male patients.

- EVL (expansion vibration lipofilling): This fat grafting technique, also developed by Dr. Wall, Jr., repurposes the fat removed with SAFELipo to strategically build up the chest and, often, the triceps and shoulders.
- Surgical excision: When necessary, our surgeons remove excess glandular tissue through tiny incisions concealed along the edge of the areola.
Special considerations for massive weight loss patients
In rare cases, such as patients with significant skin excess after massive weight loss, skin removal and tightening may be required. If our surgeons remove excess skin, they carefully plan and camouflage the scars along the natural outline of the chest musculature.
Recovery After Gynecomastia Surgery
Most patients find recovery straightforward, with manageable discomfort and a relatively quick return to daily activities; however, experiences vary with the specifics of the surgery and individual patient factors.
Postsurgical Pain
Discomfort after male chest enhancement is typically very well tolerated. Our enhanced recovery protocols and medications usually eliminate the need for narcotic pain medication. Most patients do very well with anti-inflammatory medications only.
Compression Garments
Our patients wear compressive foam and a compression vest for 4 weeks to support healing and enhance definition.
Returning to Daily Activities
Most patients return to non-strenuous activities within 24 to 48 hours and strenuous exercise in about 2 weeks.
