Revision & Complex Breast Surgery
Our surgeons perform complex, corrective breast surgery to revise the results of a previous breast enhancement procedure, typically to fix cosmetic issues including asymmetrical breasts, fold malposition, or unnatural looking surgical results. They can also correct congenital deformities or special problems that affect the appearance of the breasts. The challenges presented by these surgeries – scar tissue, reconstructing the implant pocket, compromised blood supply or tissue quality, previous incisions, significant breast asymmetry, and many more – all contribute to these cases being more difficult for surgeons to perform and manage. Given their complexity, these types of cases always require an individualized approach and tailored care for each specific patient. Women travel to Louisiana from across the country for these types of procedures with our surgeons, often based on referrals by other plastic surgeons or physicians.

Case 207: Redo Surgery/Problem Cases
This 46 year old woman actually had breast augmentation and breast lift in 2004 with poor results. She had 375 cc saline implants placed at her first surgery. Her breasts STILL drooped over her implants and had a long shape. She also did not like the size of her large areolas. We discussed her options and she wanted to change to the cohesive gel implants with another lift, reducing the size of her areolas. I placed 450 cc moderate profile Gel implants subpectorally with a lift. On the left side, I removed 252 grams of skin and breast tissue. On the right breast, I removed 236 grams of skin and breast tissue. She is ecstatic now. This procedure was performed at our certified Plastic Surgery center located in Shreveoport, Louisiana.
Case 208: Redo Surgery/Problem Cases
This patient had 25 year old liquid silicone gel implants that had ruptured and caused scar tissue thickening - or capsular contracture. She wanted to maintain her volume, but have the implants in a more natural position. In surgery, I removed the thickened scar tissue around her old implants and placed Mentor smooth round cohesive gel implants in her previous subpectoral pocket. She obviously looks and feels tremendously better! This procedure was performed at our Louisiana plastic surgery center.
Case 211: Redo Surgery/Problem Cases
This 33 year old woman suffered with significant breast asymmetry, making it very difficult to buy bras or wear swimsuits. She had 4 children which she breastfed. I performed an augmentation with breast lift to improve her symmetry. In these type of presentations, saline implants are wonderful to get good volume symmetry. Because they are filled once they are inside of the patient, it is much easier to determine the right volume. On the right, she has 500 cc and on the left breast 450 ccs. I removed only 16 grams of breast tissue from the right and 75 grams from the left breast. Now she is a pretty 36 D and looks amazing in a swimsuit! This breast lift was performed at our certified surgery center located in Shreveport, Louisiana.
Who Can Benefit?
Women who present with unique breast enhancement challenges should seek the services of a surgeon who specializes in complex and revisionary breast surgery. Corrective breast surgery procedures are usually performed to improve or correct problems resulting from prior breast enhancement procedures. They are also performed on patients who were happy with their original breast surgery, but now have age-related changes and want to continue to look their best. These surgeries are also performed on women with congenital breast deformities, significant breast asymmetry, or other unique challenges.
Problems seen from previous breast augmentation and breast lift procedures typically include:
- Capsular contracture (hardening of the breasts)
- Implant displacement or muscle flexion deformities
- Implants bottoming out and/or "blowing out" to the side of the chest
- Uneven or misplaced inframammary folds (fold malposition)
- Symmastia or "bread loafing"
- Visible wrinkling or rippling
- Implant rupture or leakage
- Inappropriate implant size
- Too much, or not enough fullness of the upper part of the breast
- Wide, thickened, uneven, or misplaced scars
Complex breast issues that require surgery include:
- Severe breast and/or chest wall asymmetry
- A missing or underdeveloped pectoralis major muscle (Poland's syndrome)
- Pectus excavatum (a concave chest)
- Pectus carinatum (chest protrudes outward between the breasts)

Our primary goal is to have satisfied patients, so you can be assured that our plastic surgeons will give you honest treatment recommendations, whether that includes surgery or not. Preparing for Your Complex or Revision Breast Surgery
At the consultation, your plastic surgeon will review your general health and discuss the specific issues that need correction. Our goal is a safe and satisfying outcome that will last. To help achieve that goal, we require that patients are nicotine-free for a month or longer before surgery and for at least 6 weeks after surgery to ensure proper healing and reduce the array of problems associated with smoking and using other nicotine products. We also strongly encourage our patients to be at the weight they expect to maintain in the foreseeable future.

What's involved in your corrective breast surgery will depend on the condition(s) being addressed. It's important to understand that revision surgery following breast augmentation is much more involved than simply shifting the location of your breast implants or substituting new implants for your old ones. Because our goal is to provide a long-lasting solution, our surgeons use advanced surgical techniques to minimize the development of recurrent problems. They may also need to reconstruct and/or reinforce the implant pocket and address existing scar tissue. Surgery to correct complex breast problems varies depending on the issue, but it will be explained in detail by your plastic surgeon during the consultation. Many times, it is necessary to be seen by your plastic surgeon more than once prior to surgery, in order to ensure you are both on the same page regarding your specific care.
Fat Grafting — Natural fullness right where you want it
Using fat transferred from another part of your body, fat grafting can add lasting, natural-looking shape and volume.
Your procedure will be performed at The Wall Center's ambulatory surgical center, which is certified by Medicare, accredited by AAAASF, and licensed by the state of Louisiana, and is one of a select few triple-certified plastic surgery centers in the country. Depending on the extent of your procedure, we may use either local or general anesthesia, but in most cases the patient is asleep. If local anesthesia is chosen, you will receive a sedative so that you feel fully relaxed and do not experience any discomfort during the procedure.
Practically all of these procedures are performed on an outpatient basis. After your surgery, you will be monitored until awake, alert, and comfortable, then discharged. Patients and their loved ones may choose to stay overnight or multiple nights in one of our luxurious guest suites, with optional overnight medical supervision, as well as all the comforts of a luxury hotel.
Our surgeons' meticulous techniques promote quick healing, and most patients are able to resume light activities within a day or two. You will be provided detailed, specific instructions for aftercare, and your doctor as well as our Wall Center nurses will closely monitor your recovery.
